PRE-CLINIC CHECK IN FORM
If you and your furry friend are coming into the clinic soon please complete this form prior to your appointment! This allows for a quicker and easier check in process. We require a new check-in to be filled out yearly.

FEAR FREE PRE-VISIT CLINIC QUESTIONNAIRE
Download and fill out this Fear Free questionnaire to help us learn about your cat’s well-being and stress points when coming into our clinic. We would love to make every visit as fear free as possible. This questionnaire is optional but helps us greatly!

GENERAL SURGERY OR DROP OFF CONSENT FORM
If your cat is coming to us for a surgery or is staying with us for a long period of time without your presence, please complete this form. Filling out this form prior to your arrival will allow for a quick and easy drop off!

DENTAL SURGERY CONSENT FORM
Please complete this form if your cat is coming in for dental surgery. This includes anything from a minor cleaning to teeth removal!

Dear Clients,

With recent changes to restrictions on businesses, we have made some important updates to our operating policies.

1. WE CAN NOW SEE ALL CASES BY APPOINTMENT ONLY
This includes vaccines, wellness exams, blood work, heartworm testing, spays and neuters, dental services, and more!

2. SAFETY MEASURES TO KEEP EVERYONE SAFE

  • For your safety, and the safety of our staff and community, clients will not be allowed to enter the building. Upon arrival, please call us from your vehicle for further instructions.
  • Place your cat (in carrier) on the wicker table by the front door. Step back 6-feet to the designated yellow line or yellow boxes. A team member will retrieve your cat and obtain information from you. Please wear a mask at all times when engaging with our team members.
  • One of our veterinarians will examine your cat either over the phone in the exam room with large window for your viewing or as a drop off. Questions and concerns will be addressed over the phone and treatment plan arranged.
  • Continue the use of credit cards and e-transfers as the preferred payment method.
  • Continue with curbside pickup of food and medication (unless you have used our online store and are having your order delivered directly to your home). To place an order through our online store, visit our website and click on "Online Store".
  • If you are ill and/or showing symptoms: fever, dry cough, respiratory issues, chest pain or traveled in the last 2 weeks, please call us to reschedule your appointment OR have a friend or family member (not residing in your house) bring your cat in.

    3. OPERATING HOURS

    We are OPEN with the following hours:

    Monday, Wednesday, Thursday, Friday: 7:30am - 6:00pm
    Tuesday: 7:30am - 7:00pm
    Saturday: 9:00am - 4:00pm
    Sunday: CLOSED
